Due
to the nature of custom prescription sunglasses, some lenses
and prescriptions may not work with all frames. By filling
out the form below, you'll give us the opportunity to see
if your prescription will work with the frames you would like
to purchase. An RXsunglass.com tehnician will contact you
with detailed information and alternate frame choices if necessary.

During
the purchase of your glasses, you will encounter the following
forms. We require this information be complete before your
order can be processed. Some styles are not available for
certain prescriptions, but we will work with you to find a
solution:
- Sphere
/ Cylinder / Axis / Prism / Add Power (bifocal only)
- Your
doctor's contact information

| Bifocal
Lens Availability |
| Because
you are likely to wear your sunglasses on uneven or unfamiliar
terrain, and as an occasional-use bifocal, we do nto place the
bifocal as high as on your normal reading glasses. We recommend
the use of a 25mm flat top bifocal. These bifocal segments are
available in add powers between +1.25 & +2.50. Custom segment
placement requires a special request. Progressive, no-line bifocals
are not available at thsi time. |

you require bifocal lenses, we can normally accomodate you.
You will notice pricing options for single vision and bifocal
lenses.

| Placing
Your Order |
| After
your prescription information is complete, and approved, your
glasses will be manufactured. Manufacturing time varies with
certain options. Please allow at least 2 weeks for the delivery
of your glasses. All items are shipped UPS ground unless noted
otherwise. |
